The Parent Resource and Registration Center at Burnham

Welcome to the Haverhill Public Schools! We are excited to have your family and your student join our school district. Going to a new school is a very important step for your child and can be an anxious time for parents. With that in mind we want to make the process of registration as easy as possible.
 
The Haverhill Public Schools’ Parent Resource and Registration Center is located at 45 Fountain Street and was established to centralize registration and to make a smooth transition for parents and students into the Haverhill Public Schools. This building was converted into offices in 2008 and it houses the Registration Center, Special Education, Transportation and our English Language Learner departments. 
 
To schedule an appointment to register your child for school, please contact 
978-420-1951. At that time we will also gather some information to make your appointment go as quickly and smoothly as possible. 
 
At your registration appointment you MUST bring the following:
 

1. Proof of residency in the form of a current mortgage/rent receipt or current utility bill. *
2. Parent/Guardian’s picture identification
3. Your child’s birth certificate
4. Transcript or report card from the last school your child attended.  If you have difficulty obtaining this please contact us for assistance.
5. If your child receives services through an IEP or 504, copies of this plan.
6. Copy of your child’s last physical (within past 12 months)
7. Current Immunization Record
8. Lead Test for Kindergarten entry only

 
If possible please complete the registration form below and bring it with you to your appointment.  Copies of this form are also available at the registration center.

  Para hacer una cita para matricular a su hijo a la escuela, por favor llame al
978-420-1951.    En ese momento le pediremos alguna información con el propósito de hacer su cita lo más rápida y fácil posible.
 
A la cita para la matrícula DEBE traer lo siguiente:
 

1. Prueba de su domicilio, ya sea recibo de hipoteca, contrato de arrendamiento o un recibo reciente de servicios ( luz, gas o cable)
2. Identificación con foto del Padre/Tutor
3. Partida de nacimiento de su hijo
4. Expediente académico o calificaciones de la última escuela a la que su hijo asistió. Si tiene dificultad obteniendo estos documentos, por favor comuníquese con nosotros para asistencia.
5. Copias de PEI o 504, si su hijo recibe estos planes educativos.
6. Copia del examen físico más reciente de su hijo (en los últimos 12 meses)
7. Archivo reciente de vacunas
8. Prueba de plomo, para los que van a entrar a Kindergarten solamente

 
Si le es posible, por favor complete el formulario de matrícula y tráigalo con usted a la cita. Hay copias de este formulario disponibles en el Centro de Matriculas.

*Education of Homeless Students*
  
Students who are considered homeless under the McKinney-Vento Act will have the right to enroll immediately upon registration (unless lacking proper immunizations).  Homeless students also have the right, in most instances, to continue their education in their school of origin with transportation provided.   If you have any questions please contact Kevin Eldridge at 978-420-1921 or Lorna Marchant at 978-374-5730.

*Educación de los Estudiantes sin Hogar*
Los estudiantes que se consideran sin hogar (desplazados) bajo la ley McKinney-Vento tendrán derecho a  comenzar clases inmediatamente, una vez matriculados (a no ser que no tengan las vacunas necesarias). Los estudiantes sin hogar tienen el derecho, en la mayoría de los casos, de continuar su educación en la escuela de origen con transporte incluído.
